The Clinical Care Coordinator serves as a key member of the patient care team across Boulder Community Health (BCH) clinics, supporting care coordination, quality initiatives, and value-based programs. This role focuses on connecting patients-especially those with chronic conditions or high healthcare utilization-with appropriate care and community resources, while promoting return to primary care. The coordinator also facilitates smooth care transitions after emergency department visits and collaborates with interdisciplinary teams to enhance patient outcomes and reduce avoidable high-cost care.

BCH primary care physician clinics are guided by the philosophy of the Patient-Centered Medical Home. The medical home organizes care around patients, while engaging a multidisciplinary care team working proactively to coordinate and track care for each individual patient.

This position is full time (40 hours/week) Monday - Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 8:00 am - 4:30 pm MST. It may be necessary, given the business need, to work occasional overtime.

We offer on the job training. The hours during training will be aligned with your schedule.

 

Primary Responsibilities:

  • Work cooperatively and collaboratively as a member of a care team with primary care providers, MAs, nurse care managers, and other clinic staff to provide care to patients based on best practices.
  • Using department workflows and training, provide simple patient support for community resources, chronic disease topics, and care navigation. Escalate patients to licensed clinicians per protocol.
  • Huddle with RN Care Managers and Transitional Care Nurses to address barriers to patient care.
  • Run and edit daily and ad hoc reports using electronic medical record.
  • Provide emergency department follow-up calls per protocol.
  • Provide resource referrals for patients requesting assistance with social determinants of health such as food & meals, housing, home safety, vision & hearing loss, health literacy, and transportation.
  • Maintain and update shared documents and resources for the ICS team, periodically reaching out to community resources to refresh the directory.
  • Reinforce care plans under the guidance of the clinical team.
  • Schedule patients for services necessary to meet disease management guidelines including follow-up from other levels of care.
  • Provide patient education resources and tools as ordered by the provider or care team, or per policy; may require travel to the office to mail documents to patients as needed.
  • Under the guidance of the diabetes educator, may support the BCH diabetes education program with administrative tasks, patient follow-up calls, and program documentation.
  • Perform effectively in stressful and/or emergency situations.

Required Qualifications:

  • High school diploma / GED or equivalent work experience 
  • Must be 18 years or older
  • 1+ years working in a clinical setting, healthcare system, or community resource organization experience 
    Experience working with an electronic medical record system
  • Working intermediate knowledge of and ability to work independently with Microsoft Office suite: Outlook, Word, Excel, and Teams

Preferred Qualifications:

  • 1+ years of coordinating patient care in a healthcare setting
  • Knowledge of patient-centered care and the Patient-Centered Medical Home (PCMH) model
  • Knowledge of quality metrics and shared savings models
  • Well-developed oral and written communication skills

 

Telecommuting Requirements:

  • Ability to keep all company sensitive documents secure (if applicable)
  • Required to have a dedicated work area established that is separated from other living areas and provides information privacy.
  • Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service.

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you'll find a far-reaching choice of benefits and incentives. The hourly pay for this role will range from $20.00 - $35.72 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
