Position in this function is responsible for oversight of psychosocial services provided by the hospice division for patients and families served including policies and processes, clinical management, education, and orientation. This position will oversee the implementation of education, documentation and program standards.


Primary Responsibilities:

  • Assists in the development and implementation of policies and procedures that support and direct psychosocial services provided to patients and families in hospice 
  • Knowledge of clinical services provided to end-of-life patients and their families including initial consultation and diagnostic assessment with a wide range of mental health, behavioral, emotional, physical, and developmental concerns 
  • Oversight of crisis assessment and intervention including assessment for safety in the hospice setting 
  • Provides consultation for social workers regarding complex crises and mental health concerns in the hospice setting 
  • Development of educational programs and outreach to address community issues related to end-of-life
  • Liaison and resource for hospice social workers regarding the Hospice Promise Foundation process 
  • Development of on-going educational programs and competencies for hospice social workers 
  • Oversight of the orientation process for hospice social workers 
  • Knowledge of current hospice state and federal guidelines and programs 
  • Development of Social Worker work groups to discuss resources, educational needs, and development of needed processes, assessment tools, policies and procedures for social services in hospice 
  • Provides at least quarterly education for social workers 
  • Maintenance of Social Work tools in iTrain 
  • Other duties as assigned

Required Qualifications:

  • Master's Degree from a school of Social Work accredited by the Council of Social Work  
  • 5+ years of social work experience in a health care setting 
  • 1+ years of social work experience in hospice  
  • Driver's License and access to reliable transportation


Preferred Qualification:

  • Management experience  


Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you'll find a far-reaching choice of benefits and incentives. The salary for this role will range from $58,800 to $105,000 annually based on full-time employment. We comply with all minimum wage laws as applicable.
