Job Description

Job Title: Grant Manager Health Equity and Quality Improvement

Program(s): Behavioral Health

State: Arizona Remote

Population Served: Adult and Pediatric Behavioral Health

Reports To: Quality Management Director and/or Executive Director

FLSA Status: Non-exempt

SUMMARY

The Grant Manager Health Equity and Quality Improvement is responsible for the oversight and management of the Targeted Investment (TI) 2.0 grant which focuses on enhancing health equity practices at the locations of Human Resource Training, Inc. (HRT) and Pathways of Arizona. S/he is responsible for creating and evaluating organizational practices, developing programs that provide culturally appropriate services, and implementing policies and procedures. The Grant Manager pulls together upper management, technical and clinical staff where appropriate, to design systems and processes to overcome barriers to optimum member care. Additionally, s/he is responsible for performing specialized tasks such as NCQA accreditation, connecting and tracking through the statewide closed-loop referral system, tracking z-codes and outcomes for social determinants of health (SDOH), and overseeing high-risk medical clients.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Manage the TI 2.0 grant focusing on NCQA accreditation, CLAS standards, and health equity practices
  • Create policies and procedures to support health equity initiatives
  • Develop and implement programs that offer culturally appropriate services
  • Oversee the closed-loop referral system and Health Information Exchange (HIE)
  • Connect to the statewide closed-loop referral system, including creating scopes of work and generating reports
  • Track Z-codes, referrals for social determinants of health, and outcomes for high-risk medical clients
  • Review reports, provide feedback, and report outcomes for funding purposes
  • Track grant funding for both entities
  • Provide and present reports on data to staff
  • Attend relevant TI meetings
  • Market outcomes to staff and clients
  • Demonstrate leadership qualities and collaboration
  • Meet contract/grant deliverables
  • Strong organizational skills
  • Maintain professional boundaries and ethical standards
  • Complete all required training
  • Experience and effective public speaking and presentation skills
  • Timely and accurate reporting and tracking

WORK ENVIRONMENT

  • Needs adequate visual acuity, ability to grasp and handle objects.
  • Needs ability to communicate effectively through reading, writing, and speaking in person or on telephone.
  • The noise level in the work environment is usually moderate

TRAVEL

  • Will require up to 5% travel within Arizona to attend TI Meetings.

REQUIRED EDUCATION AND/OR EXPERIENCE

  • Bachelors Degree in Social Services, Psychology or related field and experience in adult and/or pediatric behavioral health;
  • Experience in managing grants, health equity initiatives, or quality improvement programs
  • Knowledge of CLAS standards and health equity practices
  • Strong analytical and problem-solving skills
  • Excellent communication and team management abilities

PREFERRED EDUCATION AND EXPERIENCE

  • Masters Degree in Public Administration, Quality Management, Compliance or related field
  • Bilingual-Spanish/English

CERTIFICATES, LICENSES, REGISTRATIONS

  • Must have current fingerprint clearance (and maintain during employment) and be able to pass the background check.
  • Proof of: current CPR/FA and a valid Arizona drivers license, current auto insurance, as well as proof of education are required.
