The Registered Nurse Navigator - High Risk Breast Program acts as a case manager, navigator and educator who serves as a consistent care coordinator throughout the continuum of care assessing the physical, psychological and social needs of our patients. Our nurse navigators serve as a single point of contact for referring physicians, patients, and caregivers to provide resources and assistance with accessing clinical and supportive care. They focus on achieving the outcomes within effective and realistic timeframes. They will monitor the patient and triage needed services across all product or service lines while ensuring proper access to these services. In addition, they will extend services to help the family learn care-giving skills and patient self-care skills. Our Nurse Navigators help enhance patient outcomes, increased satisfaction and reduced costs of care. 

Primary Responsibilities:

  • Proficiently knowledgeable about breast diseases
  • Facilitate patient appointments, including those made with labs, diagnostic modalities, and specialty physicians
  • Coordinate multidisciplinary planning conferences, collect patient information for use by the care team, and document recommendations made by leadership utilizing standardized care protocols in accordance with nationally recognized care guidelines
  • Assist in developing patient education programs and tools
  • Need to be a provider of educational information and support to all women concerned about their breast health
  • Facilitate physicians’ recommendations for patient care to help ensure patient compliance and understanding
  • Provide professional guidance in coordination, execution and evaluation of patient healthcare treatment plans
  • Addresses patients’ unmet needs and enhances patients’ satisfaction by responding to the patient requests; providing information and support during illness
  • Assess patients to help identify high-risk patients (physical, psychological and social)
  • Help navigate and educate patients through survivorship
  • Assist in planning healthcare pathways with multidisciplinary care team
  • Be an educator for other medical personnel in understanding benign and malignant diseases, their treatment and the patient’s emotional and social recovery from the disease
  • Work with cancer registry staff and others to collect data, track outcomes, and support strategic planning processes

Required Qualifications:

  • Active NY Licensed RN
  • Breast Surgery or OBGYN experience

Preferred Qualifications: 

  • BSN 
  • Oncology Certified Nurse
  • Demonstrated ability to develop and implement professional and community-based education programs

New York Residents Only: The hourly range for this role is $28.03 to $54.95+ per hour. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
