The Patient Navigator will be responsible for conducting outbound calls and answer inbound calls for LIS/MSP enrollment, off-season survey follow-up, disenrollment investigation, provide patients with community resources, close gaps on HOS, etc. for all markets as needed. The Patient Navigator will support a variety of strategic activities, based on Member Experience data and performance metrics throughout the OptumCare enterprise. This individual will maintain the highest standards to ensure the members have the best healthcare experience by facilitating the process every step of the way, and will complete daily activities reports.
Schedule: Monday-Friday (No weekends, no holidays). Must be flexible to work hours in different times zones as needed for a nationwide account.

Primary Responsibilities:
- Serves as the clinical liaison with clinical performance and administrative staff as well as providing expertise solving member issues
- Makes outbound calls to assess members' current health status, as well as take inbound calls to resolve inquiries and assist members as needed. Warm transfer to other departments or services as needed
- Assesses patient needs using established triage protocol, “concierge” style in order to coordinate appropriate level of assistance needed (pharmacy, referrals, etc.)
- Assist with completion of patient satisfaction surveys
- Manage multiple short and long-term projects

- Performs all other related duties as assigned

Required Qualifications:
- Current, unrestricted LVN/LPN license
- 3+ years of clinical experience as a LVN or LPN
- 3+ years of experience in the health industry and/or in a large complex matrixed organization
- Intermediate level of proficiency with MS Excel, Word, and PowerPoint– Project scope documents, project presentations, etc.
- Intermediate level of proficiency with SharePoint
Preferred Qualifications:
- Associate's Degree (or higher)
- Experience working in a medical care setting as a medical assistant or patient care coordinator
- Call center nursing role experience
- Bilingual language proficiency (English/Spanish)
Soft Skills:
- Excellent verbal and written communication skills
- Organized and detail oriented

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
